Analysis on Effect of the Spherical ICL Surgery on Cylindrical Refractive Power Decrease

Abstract
Purpose: The purpose of this study was to measure the changes of the cylindrical refractive power with pre- and post- spherical ICL surgery, and identify the theoretical evidence based on this results.

Methods: In twenty subjects with myopic astigmatism undergone pre- and post- spherical ICL surgery, objective refraction was performed to measure changes of spherical and cylindrical refractive power and the imaging equation was calculated for changes of cylindrical refractive power, and then the transfer matrix method was used to compare the imaging equations.

Results: The mean values of cylindrical refractive power changes of pre- and post- ICL surgery were C-1.74¡¾0.82 D, and C-0.83¡¾0.56 D, respectively, showing a decrease of 0.91 D. Based on this results, when calculated with the imaging equation of ¡¯¥ÄD¡Çeq1{1?1n(dD¡ÇICL?dD¡ÇL)+dd¡Çn2D¡ÇICLD¡ÇL}¡Â¥ÄD¡Çeq1¡¯
the cylindrical refractive power was found to be reduced or equal after the surgery. Also, it was confirmed that the results of the imaging equation are very similar when calculated by the transfer matrix method.

Conclusions: The reduction of cylindrical refractive power that actually occurred after spherical ICL surgery, could be theoretically proved through the imaging equation, and this results were very similar even if a transfer matrix method calculation was applied, which is relatively easy to calculate. As a result, we have provided theoretical evidence for cylindrical refractive power reduction that is clinically found in spherical ICL surgery or spherical contact lenses wearing.
